Dear Colleagues,
We would like to remind you of the deadline for completing the survey, collecting "EIC Detector-1 Institutional Information”
by Monday, May 30, 11:59 PM (EDT). So far, only 42 institutions have submitted their institutional information!

Please review the instructions below!
The most urgent section of the institutional survey is the first section, focusing on "General Institutional Information”.
As for all FTE-related questions in the "Institutional Research group size" and "Consortia Survey" sections, please do your best
to answer those questions. If you cannot yet, submit the urgently needed "General Institutional Information"
questions now
and complete the FTE-related questions at a later stage. We need answers to the "General Institutional Information" questions
to form an Institutional Board, which is a critical step toward the formation of a new collaboration.
It is our distinct goal to provide a first overview of an institutional list during our next bi-weekly meeting on Thursday, June 9, at
08:00PM (EDT). Please note the change of time rotating the bi-weekly meeting between Friday, 10:30 AM (EDT) and Thursday,
08:00 PM (EDT), as announced during our last bi-weekly meeting.

The survey form should be completed by each institution only once. Please consult with your institutional representative tocollect the requested information. The survey will collect institutional information for both existing and new institutions.

The survey is separated into several sections:
(A) General Institutional Information (Institutional representative, Name, Address, Total size of research group by category, Consortia Membership)
(B) Consortia Information (Size of the research group by category involved in consortia)
(Silicon Consortia, AC-LGAD Consortia, EEEMCal (Endcap) Consortia, PID Consortia, Other Consortia)
(C) Physics and Sub-system Interest
(D) Resources and Infrastructure (Lab/Computing/Machine shop)
(E) Comments and Feedback
Note: Labor estimates (Size of research group) should be estimated in units of Full-Time Employees (FTE) per year for the following labor categories, assumingthe guidelines below for yearly fractional FTE efforts:
- # Annual FTE Faculty: Max. 0.25- # Annual FTE Scientific Staff / Research Professor: Max. 0.5- # Annual FTE Postdocs: 1.0- # Annual FTE Graduate students: Max. 0.75- # Annual FTE Undergraduate students: Max. 0.2- # Annual FTE Mechanical Engineers: Max. 0.8- # Annual FTE Electrical Engineers Max. 0.8- # Annual FTE Mechanical Technicians/Designers: Max. 0.8- # Annual FTE Electrical Technicians/Designers: Max. 0.8
Example: If your institution has two faculty members and you assume a yearly fractional FTE effort of 0.2 each, then 0.4 should be enteredfor # Annual FTE Faculty at the appropriate sections of this survey.
