Dear Don and Ani:

Attached please find our summary for the “Phases of QCD Matter” Town Meeting in Philadelphia (Sep. 13-15, 2014), in its final form. Please forward the document to the members of the Long Range Plan Working Group and the DNP Executive Committee. In a couple of days it will also be available from the arXiv.

A first complete draft of this document was circulated on Jan. 4, 2015, to all participants of the “Phases of QCD Matter” Town Meeting as well as a few colleagues that we knew had not been able to attend the meeting. The 2-week public comment period ended on Jan. 18, 2015. A significant number of highly constructive comments were received and incorporated by us early this week into this final document. On Wednesday the polished draft was, once again, circulated to all authors and commenters for final approval. We are confident that the attached document reflects the consensus of the entire U.S. “hot QCD” community.

We hope that the LRP WG will find this summary useful and stimulating in their upcoming deliberations of the 2015 Nuclear Physics Long Range Plan.
