  Dear sPHENIX Users at BNL,
     There will be a safety stand down in all CAD facilities tomorrow,
    Friday October 22. The only work to be done tomorrow in CAD
    facilities including 1008 and 912 are safety discussions. This
    applies to employees and visitors working in CAD facilities.
    There will be a CAD All Hands Safety meeting tomorrow.
    Anyone wishing to participate
    in the CAD All Hands Safety meeting tomorrow can attend the
    Zoom meeting with the connection information below.

Subject: C-AD wide safety stand down tomorrow, October 22nd, 2021

Our department has now clearly fallen behind on safety.  The number of safety events are visibly increasing.

Most urgently, we had 3 electric shock events since July 2021 including a shock event today.

In these 3 events, 5 of our coworkers received a shock. Though nobody was seriously injured in those shocks 
the response to these events was inadequate.

 

Therefore, I am ordering a 1-day long C-AD wide safety stand-down for Friday, October 22nd, 2021,

during regular work hours 9am to 5pm.

 

We will start with a C-AD all-hands meeting at 9am:

After that I ask that all supervisors and workers review work planning, including LOTO, and responses to events
in their work areas.

All work in all C-AD facilities should pause, including beam operation, unless it is unsafe to do so.

All meetings scheduled for tomorrow should focus on safety or be canceled.

We will provide more information tomorrow.

We now need to step back and assure ourselves that all work is properly planned, can be

safely executed, and is stopped when unforeseen and unsafe circumstances are encountered.

 

I count on everyone’s active participation in tomorrow’s events. Without working safely we cannot fulfill our mission
of building and upgrading accelerators, and delivering beams to our users. The department has delivered

exceptionally well on the science mission even during the pandemic - we must and can do the same for safety.
