Attached is the box (for LOTO locks) picture that we are talking about.  Carl Schultheiss just put his lock (in addition to CAS) before he went to hipot.

Hello Russ and all,

 

45 minutes ago or so, I “caught” Pablo Rosas, Tim Costanzo et. al. before they drove to 1008B mezzanine to continue the short-circuit test.

And I’ve also found Jon Sandberg.   

 

Pablo told me that a formal LOTO procedure would take weeks and he’d get approval from Jon to write a temporary one.

Jon agrees and he’ll talk with Pablo to have this LOTO procedure as soon as possible (even though Pablo is busy).

This morning, when I said to Pablo to write a “simple” LOTO procedure, he laughed at me with the CAS Watch colleagues and said “no LOTO

I received this sPHENIX LOTO justification memo written by Jon Sanberg from Mike Danny late Friday afternoon.  I had been emailing Mike Gaffney and Mike Danny about the Magnet Mapping work permit and trying to find a way to simplify safety controls on our work around sPHENIX once the power supply is connected.  

Jon Sanberg wrote up this attached LOTO justification and scenario.  We still need a complex LOTO procedure written by a qualified person.

This is only temporary, in place until mapping is done and we can disconnect the power supply and also get outside the USI "bubble".

That may be the easier way if there is one output valve.   

With the LOTO issue, this is a compliance to OSHA and I would advise to get the expert involved in determining if it falls under Service and Maintenance or Operational.  Mike Dany know Eric Johannsen and before BSHO gets involved, it would be good to have an answer.

Ok thank you for reminding about the "don't get tangled in the robot" hazard.  That's a good catch.  Do you think we need to LOTO the compressor?  This may not be practical because we need to connect air lines and see how things are working when the robot is sitting out on that extension shelf.  People will need to be able to work on the robot with air pressure to it.  We could lock out the compressor for when people are in the bore if the robot gets stuck inside there for some reason.  Let me ask the CERN guys what they have done at other installations.

The Complex LOTO scenario is a little confusing to me too, and I think it's overkill to be honest.  I understand what Ray is going for but I really think if we lock out the power supply and have Mike Rau or Joel or another qualified person just hold on to the lock, we will be in good shape.  We are really setting ourselves up for a misstep and a violation because of the number of people involved.  On top of the this, the pole tip doors have interlocked limit switches.  When the pole tip doors are open, the magnet can't turn on.  Ray doesn't think this is sufficient for human safety, just machine protection.  I need someone qualified to write this Complex LOTO procedure which will clarify things.

Under Dosimetry/Monitoring:

TLDs may be required.

Typically, area ODH monitoring/alarm systems isn’t in this section unless POMs are required.  The ODH alarm system is identified in the Safety and security concerns. (however, cryogenic may want to be checked if there is a leak, staff may be exposed to the cryogen)

 

Prior to energizing the magnet, a physical check of all unsecure objects that could be affected by a magnetic field (this would include fasteners, tools, compressed gas cylinders) are removed.  I verified that the air compressor tank is secure and should not be an issue.

 

The hazard of robot motion when staff in bore needs to be address (including entrapment/entanglement) and mitigation documented (should be noted in the Safety and Security Concerns section of the work permit).

 

I do not understand “operations lockout” style lock.  It needs to be verified that this is the appropriate control level and not a “Service and Maintenance” activity which is covered by the LOTO subject area.

 

When will someone 5G magnetic fields be taken? The mapping station is a concern.

Attached is a revised version of the magnet mapping installation and operation work plan.  

After a few discussions with Ray Fliller and others we've added the requirement for a Complete LOTO for workers accessing the sPHENIX bore once the magnet power supply is connected.  I need to get a complex LOTO procedure written by a qualified person and I don't have that yet.

We want to start installing mapper mechanical equipment in the bore before the power supply is connected. I'd like to get this green sheet work permit approved with the understanding that we will revise it once we get the complex LOTO procedure in hand.  We will also not do this work in the bore if we don't have the LOTO procedure (and trained and briefed people) once the power supply is connected. Can you please help me get this distributed and signed off?  We want to start installing this equipment next week once we get the pole tip doors open.

The permit also covers other hazards like ODH that are already present.

There is a separate operating procedure for the sPHENIX magnet, managed by Kin Yip. I have to update this work plan with that document number reference once I get it tomorrow.

Please review and comment and let me know if you have any questions, concerns, edits, updates, or things you want to talk about.

Go ahead and distribute this to others as well for review.

We should work to get this approved and signed off by 10/10.
