Hi Ming,

 

I hope all is well.

Thank you so much for your patience with the approval process.  This email is to inform you that the request for onsite access to BNL for the MVTX Team2 listed below has been granted for the period of 3/4 – 12/31/22.  Please remind them of the following:

 

• Collaborator must take all necessary safety training required according to BNL Safety and sPHENIX protocols.
• Collaborator must adhere to all BNL COVID guidelines.
• Collaborators must adhere to all New York State COVID guidelines as set out on the website.

 

Note that it is requested all guests are required to arrive at the Lab site Monday through Friday between 7:30 a.m. and 4 p.m.  

 

1. All are required to Validate your Vaccine with BNL’s Occupational Medical Clinic (OMC)  prior to your arrival at BNL.  You may choose to contact them by email prior to arrival  OMC AT bnl.gov  or by phone 631-344-3670  to schedule an appointment.
