My guess would be - this is one of the pilots.
All fibers are polished by tech who is well over 80 and 30 years ago polished 15k fiber bundles in PbSc of PHENIX. UNIPLAST and work keep him alive. He believes he is the best for his job .... and normally his work matches his believes. But he has his bad days. To be on the safe side everything is checked one more time by girls while packaging - viewing bunched fiber ends (connectors) all at once. Dead fibers are trivial to identify, badly polished may go through. This one found is one fiber out of 25x160.
At this rate we may expect 3 more (one in OHCal, 2 in IHCal). Please pay attention and give Vladimir Stepanovich a break.
Just comment - LED system as designed says nothing about tile quality - if tile quality is questioned - we need tile tester at BNL (most of parts are there, need JH & EoB to agree and release Mike Lenz from what he is doing now for a week).

Hi Steve,
This is just a one-off bundle where, I guess, Uniplast forgot to cut off the fibers at the ferrule end and polish them.—One in more than 160 we have used so far, which were all fine.—We could have cut an polished ourselves, but we thought Sal could probably do a better job than us. I think that’s how that bundle made it to Sal. So in summary, no reason for concern.
